Provide an original birth certificate or birth card, a certificate of Baptism with parent affidavit, insurance policy on the child's life in force for two years, Bonafide Bible record with parent affidavit, passport or certificate of arrival in the US showing age of the child (FM-6670), transcript of school records of at least four years prior that states the date of birth, or an affidavit of age sworn by the parent and a certificate of age signed by a public health officer (FM-4681).

Health Immunization Forms Upload

Parents must submit two documents as part of their registration: Florida Certification of Immunization (DH-680)State of Florida School Entry Health Exam (DH-3040), including proof of tuberculin screening, reading of the test, and appropriate follow-up.

Student Registration Packet

Parents/Legal Guardians must download, complete, and upload the Student Registration Packet. The registration packet includes the following forms: Home Language Survey Form (FM-5196), Emergency Student Data Form (FM-2733), Disclosure at Time of Registration (FM-5740), Project UP-START Student Questionnaire (FM-7378)

Student Records/Transcript Upload

Parents/Legal Guardians must request official student records (transcripts and/or report cards) from their current school. Transcripts must bear the school seal and must have an original signature from the Registrar of the school. Records provided must include a minimum of three years of records inclusive of the current year. If final grades/transcripts are not available for the current year, the most current record must be uploaded and final transcripts/records must be provided prior to the start of the subsequent school year.

ADMISSIONS PROCEDURES
- Applications are processed in the order in which they are received.
- Only completed applications will move forward to random selection. Submission of a completed application, including all required support documentation, by the designated deadline does not guarantee enrollment into the program.
- Upon submission of a completed application, including all required support documentation, by the designated deadline and fulfillment of any applicable eligibility requirements, students will be admitted on a random selection basis with priority provided to non-public school students, followed by charter school students, followed by public school students who do not attend a school with a driver's education program, followed by all other public school students.
- If accepted, the student and parent will receive a program acceptance notificatoin via email after the random selection process has completed.
- The Application Deadline is Friday, May 29th, 2026 by 11:45 PM
- Acceptance Notices will be sent by June 10th, 2026.
- Program Dates: June 15, 2026 through July 21, 2026 (8:00 am to 1:00 pm on Mondays through Thursday Only).
